What we do?

  • The Childrens Disability Network Teams provide services to children and young people who are considered to have complex needs arising from a disability and the requirement for interdisciplinary working. Click here to find out how to make a referral.

  • The CDNT’s can support a child from 0-18 years of age and children with many different types of disability – physical, sensory, intellectual etc. Access to the team is based on the child’s needs at a specific point in time. A specific diagnosis is not required.

  • Some children with a disability may require access to a CDNT across their childhood, whilst others may only need the services of a CDNT for a period of time. Children with a disability often have their needs met by their local Primary Care Team, and may not always require the support of a CDNT.

  • The SMH CDNT’s work collaboratively with our colleagues in other services, such a Primary Care, Children and Adolescent Mental Health Services and Childrens Hospital Services, as and when the need arises.

The services provided to any individual child are individual to the supports they currently require to help them participate in their lives, family priorities and clinical needs.
