How do we do it?
Progressing Disabilities Services for Children and Young People is a national programme that lead to the establishment of the SMH CDNT’s in 2021. There are 12 Principles which inform the delivery of services by CDNT’s, and underpin all policies and procedures.

Our Model of Service
Our model of service delivery is Family Centred Practice. Family Centred Practice recognises families as the experts on the needs of their child and family. This allows the service to focus on what is important to the child and their family. Fundamentally, we understand our services exist to support children and families to achieve positive outcomes.
How do we do this?
- We aim to work in partnership with you and your child, and the broader community of the families we serve.
- We will provide you with information that is specific to your child’s disability
- We will provide general information that is relevant to your child and family
- We make every effort to provide coordinated and comprehensive care
- We are committed to respectful and supportive care.
What does this look like?
We work in partnership with you and your child and the broader communities of families we serve.
- We collaborate with you and your child to develop an Individual Family Service Plan when your referral is opened to the team (more about Individual Family Service Plans here)
- Every network team has a Family Representative who is elected by families.
- Every network runs a Family Forum meeting three times a year in partnership with the Family Representative.
We provide general information that is relevant to your child and family
- Every team will send you a newsletter by email three times per year. This newsletter will include updates from the team, in addition to information which is relevant to many children and families we support.
- We will update this website with information on community resources which are of interest to many families.

We provide you with information which is specific to your child’s disability
- We will work with you and your child to understand their unique strengths and challenges as they grow, before and during intervention with our team.
- We will work with you and your child to develop goals for intervention that reflect both your priorities, your childs development and their needs arising from their disability.
- We will support you to make informed decisions about your intervention from the CDNT.
- We will share information on community supports or initiatives which would be of benefit to your child.
We make every effort to provide coordinated and comprehensive care
- The team will coordinate as best as possible.
- The team will allocate a keyworker to your child.
- The team plan their interventions to be responsive to the needs of as many children as we can, and will maximise the best use of our resources
- The team will coordinate with others involved in your childs care when needed, eg Paediatrician, HSE Disability Manager, your childs school.

- We strive to embody respect, kindness, and honesty in our daily work.
- We are transparent about what CDNT can offer and our limitations.
- We actively listen to your feedback, whether compliments or concerns.
